Attention Parents of Sixth Graders:  Pink Confidential forms were passed out this week for parent signatures.  These forms are important and need to be signed under FERPA regulations.  Our nursing office has taken the medical information received from your elementary school and we want to be sure all data is correct and updated.

Attention Parents of Seventh Graders: Exclusion notices will be sent home today for those students who have not presented documentation for their 7^th grade NH State Immunization requirement Tdap Vaccine.   Please check with your PCP to see if you child has received this mandated vaccine and if not please make arrangements for them to receive it ASAP.  An appointment date will also save them from exclusion also.  Final exclusion from school will be Friday October 7^th.  Please do not hesitate to notify our office if you have any questions. 

FLU Clinic at Elm Street Middle: Our annual Flu Clinic sponsored by Nashua Public Health Department will be held on Tuesday September 27^th from 9:00 to 11:00.  Please print the online form and have your child give it to the School nurse.
